ASCEND NEXT GEN LEAGUE LIABILITY WAIVER AND RELEASE FORM
NOTICE: THIS IS A LEGALLY BINDING DOCUMENT. READ CAREFULLY BEFORE DIGITALLY SIGNING.
This Liability Waiver and Release Agreement (“Agreement”) is entered into by the undersigned participant (“Participant”) or their parent/legal guardian (if under 18 years of age) in consideration of being permitted to participate in Ascend Next Gen League events and activities, including but not limited to training, sparring, competition, and all related activities (collectively, “Activities”).
By signing this Agreement, Participant (or their parent/legal guardian) acknowledges, understands, and agrees to the following terms:
1. ASSUMPTION OF RISK
I acknowledge that participation in Ascend Next Gen League Activities involves significant risks, including but not limited to serious bodily injury, permanent disability, paralysis, or death. These risks may arise from factors including but not limited to the actions of other participants, the condition of the premises, equipment failure, or negligence of others.
I knowingly and voluntarily assume all risks associated with participation in these Activities, whether known or unknown, foreseeable or unforeseeable, and accept full responsibility for any resulting injuries or damages.
2. RELEASE OF LIABILITY AND WAIVER OF CLAIMS
To the fullest extent permitted by law, I, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release, waive, discharge, and hold harmless the following entities and individuals (collectively, the “Released Parties”) from any and all claims, liabilities, demands, actions, or causes of action, whether in law or equity, arising from or related to my participation in the Activities:
• Raul Diaz
• Ascend World Series, LLC
• Ascend Next Gen League
• Blake Grice
• Electric City MMA
• All Ascend Next Gen League event staff, coaches, trainers, volunteers, officials, medical personnel, and representatives
This release applies to all forms of negligence, including but not limited to ordinary negligence by any of the Released Parties.
3. HEALTH ADVISORY AND FITNESS TO PARTICIPATE
I affirm that:
• I am in good health and have no medical condition that would impair my ability to safely participate.
• I have consulted with a physician regarding my ability to engage in physical combat sports, if necessary.
• I understand that combat sports involve inherent risks, including but not limited to concussions, fractures, cuts, bruises, and other injuries.
• I agree to immediately inform event staff if I feel unwell, injured, or unable to continue at any point during the Activities.
4. MEDICAL TREATMENT AUTHORIZATION
I authorize Ascend Next Gen League and its staff to administer or obtain emergency medical care for me if deemed necessary. I understand that I am solely responsible for any medical costs incurred as a result of my participation.
5. VOLUNTARY EXECUTION
I acknowledge that:
• I have read and understood this Agreement in its entirety.
• I am signing freely and voluntarily, without coercion.
• I understand that by signing this Agreement, I am waiving certain legal rights, including the right to sue.
• If signing as a parent/legal guardian, I accept full responsibility for my child’s participation and the risks involved.
6. GOVERNING LAW AND SEVERABILITY
This Agreement shall be governed by the laws of the state in which the event is held. If any provision is found to be unenforceable, the remaining provisions shall remain in full force and effect.
